I am the parent or legal guardian of the athlete being registered. I understand that combine testing, including sprints, jumps, agility drills and position work, involves physical exertion and a risk of injury. I confirm the athlete is in good health and medically able to take part.
On behalf of myself and the athlete, I release Schuman Enterprises LLC DBA NUC Sports / Keystone Athletic Holdings, its staff, coaches, venues, sponsors and partners from claims arising from participation, except in cases of gross negligence. I agree to abide by the decisions of NUC staff regarding the athlete's ability to safely participate.
I authorize NUC staff to arrange emergency medical care for the athlete if I cannot be reached, and I accept responsibility for any resulting costs.
I agree that photos, video and testing results from the event may be used by NUC Sports, including on the athlete's public profile and results pages.
I agree to abide by all rules of the combine.
